No statistically significant differences were observed between experts’ specialties.</p> <p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">*Note: For full traceability of each item, the original numbering used in the Delphi questionnaire is maintained.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The Spanish Guidelines for Asthma Management<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> defines severe uncontrolled asthma as poorly controlled asthma despite treatment with a combination of inhaled glucocorticoids and high doses of long-acting β2 agonists in the previous year or oral glucocorticoids for a period of at least six months.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In Spain, asthma affects two million people<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> and it is estimated that 70% of asthma patients have poorly controlled disease,<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> a fact which points to the need for alternative therapies. The most innovative options are biologic therapies,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> with omalizumab being the first biologic therapy approved for this disease.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Other biologic therapies with a different mechanism of action have subsequently been approved with an indication for severe refractory eosinophilic asthma; the first treatment targeting IL-5 was mepolizumab, followed by reslizumab. Benralizumab is directed against the alpha-chain of the interleukin-5 receptor and dupilumab a dual inhibitor of IL-4 and IL-13.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8–10</span></a> These treatments have a significant impact on symptom control in severe asthma patients, reducing the rate of exacerbations and the dose of oral glucocorticoids while improving quality of life.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Initially, these treatments were administered exclusively in the clinic.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> However, the approval of prefilled syringes and autoinjectors for at-home administration has brought about a substantial change in the treatment of severe asthma, offering patients a choice about where to administer the treatment. Self-administration at home has several advantages, such as flexibility in scheduling and portability, and potentially minimizes healthcare costs by reducing clinic visits.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> However, according to healthcare professionals (HCPs), not all patients are optimal candidates for at-home administration. There are questions about not only to the patient’s ability to self-administer the drug at home with no supervision, but also other aspects that may lead to suboptimal asthma control and complications. The decision as to whether a severe asthma patient is an appropriate candidate for the at-home administration of a biologic therapy is based on individual, clinical, and subjective criteria, meaning that consensus-based characteristics need to be established.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The COVID-19 pandemic challenged the pre-pandemic decision-making process. The inability to conduct face-to-face follow-up visits or administer therapy in the clinic coupled with the fact that patients with severe asthma have risks associated with poorer COVID-19 outcomes precipitated decisions forced by the situation rather than by a consensual doctor-patient decision. Two years after the start of the pandemic, the situation must be reassessed, the decisions made must be re-evaluated, and the lessons learned must be leveraged moving forward. HCPs still need tools to help them decide about whether to switch to at-home treatment based on the patient’s situation. The main objective of this work was to develop an expert-consensus-based document to establish the characteristics of severe asthma patients on biologic therapy who would be appropriate candidates for at-home administration.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A scientific committee comprising ten experts from four specialties (pulmonologists, allergists, nurses, and hospital pharmacists) developed a Delphi questionnaire consisting of 30 statements. A bibliographic search and review of the relevant literature were performed to prepare the questionnaire. The results of this search were used to produce the list of statements, which were organized into five topics: 1. Patient clinical characteristics, 2. Treatment adherence and the patient’s or caregiver’s ability to administer the drug, 3. Patient self-care, 4. Relationship with the healthcare professional and, 5. Patient preference and access to hospital. Each statement corresponded to a characteristic of severe asthma patients that may affect the decision on whether a patient is a candidate for at-home administration of their biologic therapy. Statements were carefully formulated to detect subtle nuances that can help in the precise definition of a candidate for at-home administration; this entailed a slight but necessary degree of overlap between items and sections.</p><p id="par0030" class="elsevierStylePara elsevierViewall">By using a modified Delphi method,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> an extensive panel of severe asthma experts that included allergology and pulmonology physicians, hospital pharmacists, and nurses answered a two-round questionnaire to define the characteristics necessary for patients with severe asthma on biologic treatment to be candidates for at-home administration. This modified method maintains all the advantages of a Delphi: controlled interaction between the panelists, discussion and reconsideration of each expert’s own opinion, and statistical validation of the resulting consensus (thus eliminating opinion biases).<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">The panel of experts, comprising 131 HCPs from four disciplines (Pulmonology: 43, 32.8%; Allergology: 35, 26.7%; Specialized Nursing: 29, 22.2%; Hospital Pharmacy: 24, 18.3%), was selected on the basis of specific, rigorous inclusion criteria for each specialty as defined by the scientific committee: at least two years of experience in severe asthma units for nurses and hospital pharmacists and five years for pulmonologists and allergists; previous experience with biologics; and, in the case of pulmonologists and allergists, at least 5%–10% of their patients diagnosed with severe asthma.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Two successive rounds of participation were required and consisted of a structured questionnaire completed on an online platform. The panelists had to log on to a platform to access the questionnaire. In the first wave (December 2019–January 2020), they filled out the entire questionnaire. In the second wave (March–April 2020), the results of the first wave were displayed and they only answered again regarding the items where consensus had not been reached. In this second wave, the prioritization question was added.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Finally, the results were analyzed and interpreted. The panelists rated the extent of their agreement or disagreement with the statement on a 1-to-9 Likert scale (1–3, disagree; 4–6 neutral; 7–9, agree). Consensus was defined as when at least 70% of the panelists gave an item the same score (disagree, neutral, or agree).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The data were analyzed overall and according to specialties. The Bowker test was used to compare both rounds. Statistical significance was established as a two-tailed value of 0.05.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Characteristics of the panelists and their overall view of at-home administration of biologic therapies for severe asthma</span><p id="par0055" class="elsevierStylePara elsevierViewall">A total of 134 healthcare experts in severe asthma were recruited, 131 of whom participated in both waves (see Supplementary data). The average number of severe asthma patients treated per month was 89, 36.3% of whom had severe asthma and almost half of whom were in treatment with biologic therapy.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Regarding at-home administration formulations, the panelists considered that the at-home administration of a biologic therapy should not be limited to a specific group of patients, although neither should it be initially administered at home to all patients who require a biologic therapy (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). In fact, 82.1% of the participants thought it should only be considered in patients in whom biologic therapy had already proven to be effective and safe. There was broad consensus on the benefits of at-home administration; it would constitute a positive change in asthma management (78.4% of the panel agreed), it would be a regular treatment for severe asthma (76.9% agreed), and the advantages would outweigh the disadvantages (75.4% agreed).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Characteristics of patients who are candidates for at-home administration of biologic therapy</span><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> presents the results obtained upon analyzing the answers given by the panelists about the characteristics of patients with severe uncontrolled asthma who are candidates for at-home biologic therapy.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Consensus was reached on all clinical characteristics except for on patients with a recent diagnosis of severe asthma to whom a biologic therapy is prescribed for at-home administration before they have been receiving hospital treatment for 12 months (42.5% agree). A candidate for at-home treatment is a stable severe asthma patient who has already benefitted from a biologic therapy, whose symptoms are controlled, who leads a reasonably normal life, who detects any deterioration in their disease, and who can identify exacerbation triggers (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0075" class="elsevierStylePara elsevierViewall">The degree of consensus on treatment adherence and the patient’s or their caregiver’s ability to administer the drug was >88%, with a score of 7–9 in the proposed items related to knowledge and skill in self-administration, the patient’s willingness to follow their physician’s instructions, and the correct use of inhalers (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0080" class="elsevierStylePara elsevierViewall">In regard to the characteristics required for self-care in severe asthma (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), the level of agreement among experts was high on all statements (≥91%, Likert score: 7–9), although a final consensus was not reached on patients who require the support of a caregiver for the management of severe asthma (25.2% agree on wave 1 versus 33% agree on wave 2; <span class="elsevierStyleItalic">p</span> = 0.001), since this depends on the degree of dependence and on the caregiver.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The relevance of the characteristics pertaining to the relationship between the patient and the HCP was obvious (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Asthma is a chronic disease and the bond generated lasts over the long term.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Finally, <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> presents the panelists’ opinion about patient preferences and how easy it is for them to access the hospital. Consensus was high (>88%, Likert score: 7–9) on considering at-home administration of the biologic therapy in patients who have difficulty accessing the hospital or those who state a preference for at-home use, aspects which may also have a positive impact on their quality of life.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Selection and prioritization of the most relevant characteristics of patients who are candidates for at-home biologic therapy</span><p id="par0095" class="elsevierStylePara elsevierViewall">Once they had stated their level of agreement, the panelists were asked to choose and classify according to clinical criteria the ten most relevant characteristics for determining that a patient is a candidate for at-home administration. There was no clear order in terms of prioritizing the different characteristics (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), leading the scientific committee to acknowledge the involvement of multiple characteristics in the decision-making process.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">At this point, a new joint analysis of the data was performed to establish which variables of those chosen as relevant were high-priority (characteristics indicated as the top three by the greatest percentage of panelists), medium-priority (4th and 7th place), or low-priority (in the last three positions). Patients who follow their physician’s indications, whose disease is stable, and who are on biologic therapy and are able to detect an exacerbation were the most important conditions for a patient to be considered a candidate for initiating at-home administration of a biologic therapy (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). No statistically significant differences were observed among the panelists’ four specialties.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Once the relationship between the degree of relevance associated with each item and the order of priority assigned to them had been verified, a combined prioritized analysis weighted by relevance was performed. Each panelist selected the ten characteristics deemed most relevant. The characteristics selected were reordered from one to ten, where one was the highest priority and ten the lowest. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> presents the mean of these scores.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">The results reconfirm that most of the consensus-based characteristics are considered in the decision-making process, although some of them carry greater weight:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">a)</span><p id="par0115" class="elsevierStylePara elsevierViewall">patient who follows their physician’s recommendations,</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">b)</span><p id="par0120" class="elsevierStylePara elsevierViewall">patient who identifies exacerbation triggers,</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">c)</span><p id="par0125" class="elsevierStylePara elsevierViewall">patient who is on a biologic therapy,</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">d)</span><p id="par0130" class="elsevierStylePara elsevierViewall">patient who has stable disease,</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">e)</span><p id="par0135" class="elsevierStylePara elsevierViewall">and, of course, a severe controlled asthma patient</p></li></ul></p><p id="par0140" class="elsevierStylePara elsevierViewall">A patient who receives a biologic therapy who meets these characteristics would be an appropriate candidate for at-home administration as per HCP criteria.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0145" class="elsevierStylePara elsevierViewall">This consensus featured the assessment of experts regarding the main characteristics of the profile of severe uncontrolled asthma patients who are candidates for at-home biologic therapy. At present, treatment administration selection should be driven by the likelihood of adherence, among other aspects. Apart from when the right time to introduce at-home administration is, the patient’s acceptance and commitment are also relevant.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The panelists agreed that at-home administration should not be limited to a specific group of patients, but neither should it be initially considered when a biologic therapy is required or until the patient has been on treatment with these drugs for at least 12 months. This is in line with the criteria recommended in treatment guidelines.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,16,17</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">Our panelists recommend at-home administration for patients who understand instructions on how to use a syringe/autoinjector; have demonstrated their ability to do so; and have no physical limitations whatsoever, since treatment adherence is a constant concern. Asthma patients’ adherence to long-term inhaled treatment has contributed substantially to asthma control and reducing morbidity, although it generally remains suboptimal. Suboptimal adherence leads to poorer clinical outcomes and increased healthcare costs.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> Nevertheless, beyond any purely clinical compliance, adherence is the outcome of a consensus-based agreement achieved through the physician-patient relationship and must be regarded as a complex, multifactorial variable influenced by the variability of human behavior.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,20</span></a> Therefore, a patient who would be a candidate is one who adheres to treatment, who regularly goes to collect their treatment, and who is capable of adapting to new routines.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The results show that there is no clear order of relevance of the items, meaning that the selection of patients who can receive at-home biologic therapy is conditioned by a combination of different characteristics and the clinician’s judgement. In view of the need for this project to produce precise recommendations, the priority of the characteristics chosen as the most relevant was weighted (>5/10 points). Healthcare professionals prioritize severe asthma patients with stable, controlled disease, who follow their physician’s recommendations, and can detect any deterioration in their disease and identify exacerbation triggers. These three characteristics must be regarded as essential for prescribing at-home biologic therapy, whereas the remaining ones are merely desirable. In this regard, there must be mutual trust between patient and physician so that the former will follow the latter’s indications and recommendations.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Although the patient must be willing to receive at-home therapy, their initial desire to do so is the aspect that was least highly rated by the panelists. This does not mean that no value is attached to the patient’s desire to collaborate or that their opinion goes unheeded, but rather that the other aforementioned premises should be evaluated first. Not all patients who seek at-home administration are good candidates, although if they fulfil the aforementioned criteria, they must then have to agree to this type of treatment.</p><p id="par0170" class="elsevierStylePara elsevierViewall">The efficacy and safety of the at-home administration of biologic therapy has been amply demonstrated.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21–25</span></a> However, some questions remain unanswered and further progress must made on them. These issues include the assessment of treatment adherence in self-administration, the method for evaluating correct patient self-administration technique, and the detection of administration-related adverse events.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">There are also some important financial considerations, since healthcare professionals have a social responsibility to choose suitable patients, i.e., those who will comply with therapeutic recommendations. Nevertheless, there are studies that demonstrate the cost-effectiveness of at-home biologic therapies and the considerable time savings achieved, not to mention the reductions in additional medication expenditure and hospital resources or expenses.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">26–28</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">To our knowledge, this is the first work that proposes clear guidelines on patient characteristics to consider when determining suitability as a candidate for at-home biologic therapy for severe asthma. This project has some limitations inherent to the Delphi method itself, such as the possibility that preconceived ideas may influence the results and interpretation of the questions, as well as the lack of quantitative questions that would allow for a more objective analysis.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0185" class="elsevierStylePara elsevierViewall">The COVID-19 pandemic prompted a series of changes in the organization and practice of care for severe asthma patients in the clinic. The recommendations established by this multidisciplinary group of healthcare professionals with extensive experience in the diagnosis, treatment, and follow-up of patients with severe asthma in Spain continue to be even more necessary. These recommendations establish disease control and the patient's ability to identify triggers for exacerbation as the most important characteristics when selecting at-home management for patients with severe asthma on biologic therapy. These recommendations could be applied directly in clinical practice and would have a high degree of impact on improved patient management.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Funding</span><p id="par0190" class="elsevierStylePara elsevierViewall">This work was funded by <span class="elsevierStyleGrantSponsor" id="gs0005">GSK.</span> Authors were in full editorial control of the content and conclusions and accepted full responsibility for the final approval of the manuscript. The at-home administration of biologics has increased during the COVID-19 pandemic, but the characteristics of severe uncontrolled asthma patients who may benefit from at-home administration of biologic therapy have yet to be identified.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">This project is based on the Delphi method, designed to reach an expert consensus through a multidisciplinary scientific committee addressing the following questions: clinical characteristics, treatment adherence, patient or caregiver administration ability, patient self-care, relationship with the healthcare professional, patient preference, and access to the hospital.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">One hundred and thirty-one healthcare professionals (pulmonologists, allergists, nurses, and hospital pharmacists) completed two Delphi consensus questionnaires. Fourteen items were identified as priority characteristics, the first five being: 1. The patient follows the healthcare team’s indications/recommendations to control their disease, 2. The patient is capable of detecting any deterioration in their disease and of identifying exacerbation triggers, 3. The patient receives biologic therapy and has stable disease with no vital risk, 4. The patient takes responsibility for their self-care, 5. The patient has occupational/educational obligations that prevent them from going to the hospital regularly.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Disease stability and control plus the ability to identify exacerbation triggers are the most important characteristics when opting for at-home administration for a patient with severe uncontrolled asthma on biologic therapy. La administración domiciliaria de biológicos ha aumentado durante la pandemia de COVID-19, pero aún no se han identificado las características de los pacientes con asma grave no controlada que pueden beneficiarse de la administración domiciliaria de terapia biológica.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Materiales y métodos</span><p id="spar0085" class="elsevierStyleSimplePara elsevierViewall">Este proyecto se basa en la metodología Delphi, diseñada para alcanzar un consenso entre expertos a través de un comité científico multidisciplinar que aborda las siguientes cuestiones: características clínicas, adherencia al tratamiento, capacidad de administración del paciente o cuidador, autocuidado del paciente, relación con el profesional sanitario, preferencias del paciente y acceso al hospital.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0090" class="elsevierStyleSimplePara elsevierViewall">Ciento treinta y un profesionales sanitarios (neumólogos, alergólogos, enfermeros y farmacéuticos hospitalarios) cumplimentaron las dos rondas de consenso del cuestionario Delphi. Se identificaron catorce ítems como características prioritarias, siendo los cinco primeros: 1. El paciente sigue las indicaciones/recomendaciones del equipo sanitario para controlar su enfermedad; 2. El paciente es capaz de detectar cualquier deterioro de su enfermedad y de identificar los factores desencadenantes de las exacerbaciones; 3. El paciente recibe tratamiento biológico y tiene una enfermedad estable sin riesgo vital; 4. El paciente se responsabiliza de su autocuidado, y 5.
