Indeed, it is the central axis of hospital medical processes and continuity of care<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">From a clinical management perspective, a piece of data that is very often used is mean length of hospital stay, which ranges between eight and 11 days in IM<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>. This should not be the only data point analyzed, given that there are patients with longer hospital stays, even longer than 30 days. The length of hospital stay can be determined by medical factors, such as comorbidities and pluripathology<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>, and other aspects such as cognitive, functional, and social factors<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–11</span></a>. Medical problems multiply as the length of the hospital stay increases; it can even cause so-called cascade iatrogenesis<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>. In addition, the length of hospital stay has an important repercussion on hospitalization expenditures<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Long hospital stays have been a constant concern in clinical care<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>. There is little information on the profile of patients hospitalized for a long period of time in an internal medicine department (IMD) and their short-term outcomes. Therefore, the aim of this work was to describe the characteristics and short-term prognostic factors in patients hospitalized for more than 30 days in an IMD over the five-year study period.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0020" class="elsevierStylePara elsevierViewall">This work is a retrospective observational study of a case series that included all hospitalized patients with a length of hospital stay greater than 30 days in a university hospital IMD from January 1, 2013 to December 31, 2017.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study setting</span><p id="par0025" class="elsevierStylePara elsevierViewall">The University Hospital of Henares, which belongs to a group of low-complexity hospitals of the Autonomous Community of Madrid Health Service, provides coverage to a population of approximately 170,000 residents. It has 212 hospitalization beds and more than 10,000 admissions and 252,000 consultations annually. The IMD is responsible for more than 50% of all medical hospitalizations at the center.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Study population</span><p id="par0030" class="elsevierStylePara elsevierViewall">All patients hospitalized in the IMD who had mean lengths of hospital stay greater than 30 days during the study period were selected<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Variables and information sources</span><p id="par0035" class="elsevierStylePara elsevierViewall">Sociodemographic variables (age and sex), prior contact with the healthcare system (hospitalizations and emergency department visits in the previous year), variables related to the hospitalization (principal diagnosis, intensive care unit (ICU) admission, and geriatric assessment), condition upon discharge (death, discharge home, or transfer to another center), length of hospital stay, and events (death or readmission) in the first 30 days following discharge were gathered. The information was obtained from the Minimum Basic Data Set (CMBD for its initials in Spanish), a set of clinical information recorded in patients’ electronic medical records in Spain.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The outcome variables were very long hospital stays (above the 75<span class="elsevierStyleSup">th</span> percentile for the study sample), in-hospital mortality, and a composite variable consisting of mortality and readmission in the first 30 days following hospital discharge.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">Quantitative variables were expressed as means and standard deviation (SD) or as median and interquartile range (IQR). Qualitative variables were expressed as number and percentage. For comparisons based on presence of a very long hospital stay (mean hospital stay in the 75<span class="elsevierStyleSup">th</span> percentile or above, in-hospital death, and adverse outcomes in the first 30 days), the chi-square test (or Fisher’s exact test, if applicable) and the Bonferroni correction were used. An adverse outcome in the first 30 days was defined as the composite variable of death or readmission for any cause in the first 30 days following discharge.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Quantitative variables were dichotomized according to clinical outcome. Variables with a <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.2 on the univariate analysis or clinical relevance (age and sex) were included in a multivariable logistic regression model by means of the enter method. Associations between groups were expressed as the odds ratio (OR) with a 95% confidence interval (CI). Statistical significance was defined as a <span class="elsevierStyleItalic">p</span> value less than .05 or if the 95% CI of the OR excluded the value of 1.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The IBM SPSS Statistics for Mac Version 22.0 statistical package (Armonk, NY) was used.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">Of a total of 11,948 patients hospitalized in the IMD during the period studied, 335 (2.8%) were hospitalized for more than 30 days (57 in 2013; 54 in 2014; 64 in 2015; 79 in 2016, and 81 in 2017). The mean length of stay for said group of patients was 45.6 (SD 19) days, with a range of between 31 and 146 days. This represents a five-fold increase (more than 37 days) compared to the overall mean length of hospital stay in the IMD (eight days).</p><p id="par0065" class="elsevierStylePara elsevierViewall">The total length of hospital stay of the 11,948 patients hospitalized in our department during the study period was 101,797 days. The total length of hospital stay for the group of 335 patients with a hospital stay longer than 30 days was 15,271 days, which represents 15% of the total length of hospital stay in the department.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The mean age was 74.5 (± 13.9) years (21% were <65 years, 19.1% were 65-74 years, 33.4% were 74-85 years, and 26.0% were >85 years of age). Of the total, 180 (53.7%) were men. A total of 257 (76.7%) patients consulted in the emergency department and 193 (57.6%) had a hospital admission in the year prior to the index episode. The three most common diagnostic groups were infectious (36.1%), cardiovascular (19.1%), and gastrointestinal (16.7%) diseases. Seventy-two (21.5%) patients had an ICU admission and 42 (12.5%) patients underwent a geriatric assessment (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Two hundred thirty-nine (71.4%) cases were discharged home; 38 (11.3%) were transferred to other centers (medium- or long-term care centers or functional rehabilitation centers), and 58 (17.3%) died during hospitalization. When comparing patients according to their destination upon discharge, differences based on the age group and whether they were admitted to the ICU during the index episode were noted (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0080" class="elsevierStylePara elsevierViewall">The median length of hospital stay was 39 (IQR 34-49) days in the study sample and 62 (IQR 54-72) days for those with a length of hospital stay greater than the 75<span class="elsevierStyleSup">th</span> percentile. Patients aged 75 years or older, patients who had consulted in the emergency department in the previous year, ICU admission, geriatric assessment, and death during the hospitalization were associated with a length of hospital stay greater than the 75<span class="elsevierStyleSup">th</span> percentile in a statistically significant manner (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). After the multivariable analysis, ICU admission (OR 2.5; 95% CI 1.4-4.7; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) and not having had a geriatric assessment were independently associated with a long hospital stay (OR 0.3; 95% CI 0.8-0.9; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.042) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Fifty-eight (17.3%) patients died during the index hospitalization. Patients aged 75 years or older, ICU admission, and a very long hospital stay were associated with in-hospital mortality in a statistically significant manner (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). After the multivariable analysis, no independent factors associated with death during the hospitalization were identified (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Of the 277 patients discharged alive, 66 (23.8%) died or were readmitted in the first 30 days following discharge (three deaths and 63 readmissions). It was noted that an emergency department visit or admission in the previous year was associated with having an adverse outcome in the first 30 days following discharge in a statistically significant manner (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). After adjusting the model, admission in the previous year was an independent factor of an adverse outcome in the 30 days following discharge (OR 2.2 95% CI 1.0-4.8) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">This work has found several main outcomes. First, 2.8% of patients hospitalized in our IMD had a length of hospital stay of 30 days or more. This subgroup of patients was responsible for 15,271 days of hospitalization, which represented 15% of the total length of hospital stay in the IMD during the five-year study period. These results are in accordance with those from the majority of IMD in Spanish public hospitals.</p><p id="par0100" class="elsevierStylePara elsevierViewall">A total of 3.2% of hospitalized patients had a length of hospital stay greater than 30 days, representing 17.4% of the total length of hospital stay<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>. In our case, unlike the study on other Spanish hospitals, the number of patients with a long hospital stay progressively increased as years passed, with all the implications this entails<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>.</p><p id="par0105" class="elsevierStylePara elsevierViewall">These results show the high degree of impact on healthcare activities—and indirectly on healthcare expenditure—of patients with long hospital stays. For example, the cost at our center is euro508/day, calculated per individual based on diagnosis-related group (DRG) and the length of hospital stay for each DRG according to the database of public prices for services and activities for the Community of Madrid. According to our center’s in-house, unpublished data, the cost per day of hospitalization in our IMD was greater than euro600/day<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>. Therefore, the subgroup of patients could be responsible for an approximate expenditure in excess of euro9,000,000.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Taking into account the impact of this group of patients, it is more than necessary to fully determine the characteristics, outcomes, and needs of these patients in order to search for alternatives to conventional hospitalization for their management and to establish their own healthcare circuits that benefit the patient and the healthcare system.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Second, this group was characterized as having an older mean age; having frequently required evaluation in the emergency department; and having been admitted for an infectious, cardiovascular, or gastrointestinal disease in the previous year. One out of every five patients had an ICU admission and one out of every ten required discharge to a continuing care center. These data are similar to those of the group of patients with long hospital stays in the IMD of other Spanish hospitals (mean age 71.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>15.5 years, 55.6% males, most common causes of admission of pneumonia and heart failure, and 10.7% discharged to a continuing care center)<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The total sample of patients hospitalized in our IMD during the study period had a mean age of 73.2 years and the most common causes of admission were respiratory, cardiovascular, and infectious diseases. The percentage of patients who required ICU admission was less than 5%, in-hospital mortality was less than 8%, and the 30-day readmission rate was 12.6%. Reviewing the DRG, it is not surprising that the most frequent diseases were infectious and cardiovascular illnesses, given that they correspond with the most common DRG found in an IMD<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Third, one out of every four patients had a length of hospital stay of 49 days or more. ICU admission and the lack of a geriatric assessment were associated with a longer hospitalization, that is, above the 75<span class="elsevierStyleSup">th</span> percentile of our study sample's mean length of hospital stay. A meta-analysis on a comprehensive geriatric assessment has demonstrated that its use reduces the length of hospital stay—more evidently in surgical than medical departments—as well as morbidity and mortality in hospitalized patients<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,17</span></a>.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Fourth, in-hospital mortality was 17% in patients with a long hospital stay and mortality in the IMD was 8%. A study by Barba et al.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> found a 9.7% in-hospital mortality rate in a group of patients with a length of hospital stay of 30 days or less and a rate of 20.8% in those with a length of hospital stay greater than 30 days. Previous studies have shown that in-hospital mortality in IMD is related to elderly age, comorbidity, reasons for admission (infection and cancer), having been hospitalized in the previous three months, and nutritional status<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18–21</span></a>. However, in our study, no prognostic factors that allow for predicting this event were found.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Fifth, among patients with a long hospital stay, 11.3% were transferred to medium- or long-term care centers or functional rehabilitation centers. Although the prevalence of hospitalization-associated functional decline varies based on the characteristics of the population, the variable’s definition, and the hospital units studied, it is estimated that three out of every ten elderly patients hospitalized in an IMD present with hospitalization-associated functional decline<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a>.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Lastly, approximately one out of every four patients had an adverse outcome at 30 days following discharge. This percentage was greater than the 12.6% found in the IMD of our center. Hospitalization in the year prior to the index event was a predictive factor of death or readmission in the first 30 days following hospital discharge. A previous study conducted in Spanish IMD showed that the length of hospital stay together with comorbidity was associated with readmission following discharge<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a>. In various risk models, hospitalization in the previous 12 months was a factor associated with readmission following discharge<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">25,26</span></a>.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Limitations</span><p id="par0145" class="elsevierStylePara elsevierViewall">This study has various limitations. First, it has the limitations inherent to its retrospective cohort design. Second, the results were markedly affected by the study period, during which multiple changes in the provision of healthcare, department personnel, and the center’s management occurred. Third, the majority of variables studied come from an administrative database, the CMBD, from which the DRG were calculated. Despite having proven to be a high-quality reflection of the real-world situation, it has recognized limitations<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a>.</p><p id="par0150" class="elsevierStylePara elsevierViewall">In addition, other variables could have been studied that would have been useful and would have provided us with more information, such as functional, cognitive, and social variables<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">28,29</span></a>.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusions</span><p id="par0155" class="elsevierStylePara elsevierViewall">Although the percentage of patients with long hospital stays is less than 3%, they have a very important impact on the routine activity of an IMD.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Compared to the totality of patients hospitalized in the IMD, the prognosis of this group of patients was worse. Among those with a length of hospital stay greater than 30 days, prior ICU admission and a lack of a geriatric assessment during the hospitalization were circumstances that increased the possibility of a very long hospital stay.
