In this context, complementary tests are of considerable use for directing and confirming the diagnosis. Computed tomography (CT) helps recognize rare conditions such as epiploic appendagitis (EA), which is an atypical cause of abdominal pain. Despite being a benign inflammatory process, EA should be considered to avoid diagnostic and treatment errors. EA Is defined as an ischemic infarction of an epiploic appendix caused by the twisting or spontaneous thrombosis of the central vein of this appendix.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Epiploic appendages are mobile, pedunculated fat structures, measuring 5–50 mm in length, surrounded by the visceral peritoneum and anchored to 2 longitudinal belts that run from the blind loop to the sigmoid (50–150 appendages in total, more abundant and larger in the left colon). These appendages have terminal vascularization consisting of 1–2 arterioles and a venule. Their functions are still unknown, although a number of authors have postulated that the appendages act as a blood reservoir.</p><p id="par0015" class="elsevierStylePara elsevierViewall">We present a series of 7 patients (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>) diagnosed with EA with various epidemiological, clinical and radiological characteristics but with a common nexus: obesity, diagnosis with abdomen contrast CT and conservative management of the clinical condition.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">The incidence of EA is unknown because most cases are self-limiting and go unnoticed due to their benign progression or go undiagnosed. EA has been reported in 2–7% of patients with an initial suspicion of acute diverticulitis and in 0.3–1% of those with acute appendicitis.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> EA occurs between the second and fifth decade of life (mean age at diagnosis, 40 years), mostly affects men (4:1) and is very uncommon in children.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In our series, however, we present patients of all ages and a similar percentage of males and females (43% males and 57% females).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The following risk factors for EA have been reported: obesity (due to an increase in abdominal adipose tissue),<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> abdominal hernia, dietetic transgression and rapid weight loss. In our series, we were able to confirm the presence of obesity in all of the patients, and only 1 of the 7 had a history of inguinal hernia.</p><p id="par0030" class="elsevierStylePara elsevierViewall">EA clinically progresses with sudden onset, intense, nonradiating abdominal pain located in the lower half of the abdomen (mainly in the left quadrant) and can simulate appendicitis and diverticulitis.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> In our series, we determined that the most common location for the abdominal pain was in the left iliac fossa (LIF) (57% vs. 43% for the LIF and right iliac fossa [RIF], respectively). Anatomically, EA can affect any segment of the colon (57% rectosigmoid, 26% ileocecal, 9% ascending colon, 6% transverse colon and 2% descending colon).<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> In rare occasions, EA can be located within a hernia sac or adhere to the vermiform appendix. However, EA does not usually progress with fever (only 1 patient of the 7 in our series), vomiting or bowel habit abnormalities, although in our series the rate reached almost 50%.</p><p id="par0035" class="elsevierStylePara elsevierViewall">In terms of complementary tests, laboratory tests do not usually show a disorder, except for an occasional slight increase in C-reactive protein and the presence of leukocytosis with neutrophilia. The gold standard diagnostic test is abdomen contrast CT, and the classical finding in the abdomen CT is an oval image of a fat deposit located in the serous of a segment of colon and parietal peritoneum, limited by a hyperattenuating ring sign<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> and, in some cases, the central dot sign,<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a> referring to the thrombosed vessel (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> In our series, abdominal ultrasounds were performed prior to the abdominal CT for 3 of the 7 patients due to suspected appendicitis, finding inflammatory changes in the mesenteric fat in 1 of the patients. In the 4 other cases, abdominal CT was performed directly due to suspected diverticulitis.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The clinical differential diagnosis includes cholecystitis,<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> twisting and rupture of ovarian cysts, ectopic pregnancy, Crohn's disease, ulcerative colitis, acute or recurrent diverticulitis<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> and appendicitis, which can occasionally progress simultaneously with this condition.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The treatment of EA requires conservative management: rest, analgesia and absolute diet at the start, with subsequent progressive reintroduction. Biotherapy, hospitalization and surgery are not necessary, except when there are complications.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> The prognosis with conservative therapy is satisfactory and leads to a rapid recovery. In our series, 100% of the patients underwent conservative management with no complications.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Lastly, we would like to point out that EA, despite being a benign condition, should be considered in the differential diagnosis of abdominal pain in emergency departments, especially in patients with obesity.
