    "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Var&#243;n de 73 a&#241;os con colocaci&#243;n de derivaci&#243;n ventr&#237;culo-peritoneal 4 meses antes por hidrocefalia normotensiva que consulta por fiebre de 39&#44;6<span class="elsevierStyleHsp" style=""></span>&#176;<span class="elsevierStyleSmallCaps">C</span> de 48<span class="elsevierStyleHsp" style=""></span>h de evoluci&#243;n&#44; presentando crisis t&#243;nico-cl&#243;nica generalizada el d&#237;a del ingreso&#46; La TC craneal mostr&#243; una lesi&#243;n ocupante de espacio en la sustancia blanca del l&#243;bulo frontal derecho de 43<span class="elsevierStyleHsp" style=""></span>&#215;<span class="elsevierStyleHsp" style=""></span>34<span class="elsevierStyleHsp" style=""></span>mm&#44; hipodensa&#44; con captaci&#243;n perif&#233;rica de contraste&#44; localizada sobre la v&#225;lvula de derivaci&#243;n &#40;<a class="elsevierStyleCrossRef" href="#fig0005">fig&#46; 1</a>&#41;&#44; asociando edema vasog&#233;nico perilesional y compresi&#243;n del ventr&#237;culo lateral derecho&#44; sin desviaci&#243;n de l&#237;nea media &#40;<a class="elsevierStyleCrossRef" href="#fig0010">fig&#46; 2</a>&#41;&#46; Se realiz&#243; drenaje del absceso retir&#225;ndose todo el dispositivo&#44; resultando los diversos cultivos positivos para SAMR&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Las derivaciones del LCR son unos de los procedimientos neuroquir&#250;rgicos m&#225;s frecuentes&#44; pudiendo ser externas &#40;temporales&#41; o internas&#46; Las infecciones se producen en el 5-15&#37;&#44; representando el 45-52&#37; de las meningitis&#47;ventriculitis nosocomiales en adultos&#44; apareciendo la mayor&#237;a en el primer mes tras su colocaci&#243;n&#46; Los factores asociados se relacionan con el paciente &#40;edades extremas&#44; proceso principal&#44; comorbilidades&#44; lesiones cut&#225;neas&#41;&#44; la cirug&#237;a &#40;duraci&#243;n&#44; experiencia del neurocirujano&#41; y con la propia derivaci&#243;n &#40;neurocirug&#237;a previa&#44; revisiones&#44; infecci&#243;n previa del <span class="elsevierStyleItalic">shunt</span>&#41;&#46; Influyen tambi&#233;n las propiedades del biomaterial&#44; los mecanismos de defensa del hu&#233;sped y los factores de virulencia bacteriana&#46; Se producen tras la colonizaci&#243;n de la derivaci&#243;n con flora cut&#225;nea durante la cirug&#237;a o por lesiones cut&#225;neas adyacentes al <span class="elsevierStyleItalic">shunt&#59;</span> tambi&#233;n v&#237;a hemat&#243;gena o por infecci&#243;n retr&#243;grada desde el cat&#233;ter&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">Predominan los estafilococos&#44; aisl&#225;ndose bacilos gram negativos en dispositivos que drenan el LCR a la cavidad peritoneal&#46; El tratamiento incluye la retirada de todo el sistema adem&#225;s de la antibioterapia&#46;</p></span>"
