In addition, insulin resistance reduces protein synthesis, contributing to the vicious circle of muscle mass loss.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Muscle mass, mainly skeletal muscle, decreases by 3% each year after 60 years of age.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> Age-related sarcopenia or primary sarcopenia <a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> becomes evident in large part if there are coexisting chronic diseases such as diabetes (secondary sarcopenia), especially in women, individuals with poor metabolic control, and those with comorbidities.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> What's more, sarcopenia is a muscle disease in which not only quantity is important, but also muscle function or strength.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Its prevalence in adults ranges from 8% to 36%. DM2 increases the risk of developing sarcopenia by 1.59<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> and its prevalence associated with DM2 ranges from 8.2% to 15.1% according to the studies analyzed.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a> Therefore, the EWGSOP-2 recommend evaluating the presence of sarcopenia through screening tests such as the SARC-F<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> and evaluating strength by means of handgrip dynamometry. The latter is the best predictor of adverse events and frailty.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In 2019, the <span class="elsevierStyleItalic">Global Leadership Initiative on Malnutrition</span> (GLIM) published a consensus document on the diagnosis of DRM based on phenotypic criteria (involuntary weight loss, low body mass index, or decrease in muscle mass or function) and two etiological criteria: decrease in intake and presence of inflammation.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> In patients hospitalized in internal medicine wards, the combination of DRM and sarcopenia is a significant healthcare problem and professionals must be aware of it.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">This work aims to analyze a cohort of patients hospitalized in an internal medicine ward to determine if there are differences in terms of nutrition and muscle between patients with and without DM2. It also aims to evaluate how both influence short- and long-term prognosis. The authors hypothesize that the combination of DRM and sarcopenia is more prevalent in patients with DM2 and that these three conditions entail a poor prognosis for these patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">This work is a single-center, prospective, observational cohort study that included consecutive patients admitted to an internal medicine department.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Population and study period</span><p id="par0035" class="elsevierStylePara elsevierViewall">In order to standardize the data from the sample, data from the first day of hospitalization were included, provided that patients met the inclusion criteria (older than 18 years with a signed informed consent form). Questionnaires on malnutrition (Mini-Nutritional Assessment-Short Form (MNA-SF))<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,21</span></a> and sarcopenia (SARC-F, which evaluates five domains: strength, assistance in walking, rise from chair, climb stairs, and falls) were administered on the first day.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Handgrip dynamometry was also performed within 24 h of admission using a Jamar 12-0604 Plus® dynamometer (ten repetitions, yielding the mean and the coefficient of variation among them) in the dominant hand.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Patients who did not sign the informed consent form, patients with type 1 diabetes, and patients who were not able to have the initial tests performed (MNA-SF, SARC-F, and dynamometry) in the first 24 h (admission over the weekend or the day before a holiday) were excluded. Patients were recruited during two periods (May and October, 2021) and received standard of care and treatment. This study was approved by the regional ethics committee prior to patient inclusion and was conducted in accordance with the Declaration of Helsinki.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study variables</span><p id="par0045" class="elsevierStylePara elsevierViewall">Anthropometric variables (age, sex, and BMI or its estimate using the Chumlea equation<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>), medical history, comorbidity (Charlson Comorbidity Index<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a>), and laboratory findings were obtained on each patient included. The results of the SARC-F scale were categorized according to a cutoff point greater than or equal to four points, which was the definition of a positive screening test for sarcopenia. Likewise, handgrip dynamometry was categorized according to the cut-off points of less than 27 kg for men and less than 16 kg for women, which determined the presence of sarcopenia in this technique.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Lastly, data were collected on discharge and the main diagnosis upon discharge as well as in-hospital death. Patients’ vital status following discharge and readmissions at 120 days of follow-up were gathered based on data from the local electronic medical record or by telephone contact between the investigators and patients or their family members.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Categorization of patients for analysis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>)</span><p id="par0050" class="elsevierStylePara elsevierViewall">Six groups based on the presence of DM2 and nutritional status were established in order to describe patients’ characteristics in detail. On the one hand, the sample was divided according to presence of DM2 and on the other hand, based on categories according to the MNA-SF questionnaire results (≤7 points: malnutrition, 8–11 points: risk of malnutrition, ≥12 points: no risk of malnutrition). Thus, six groups were analyzed (2 × 3). Qualitative and quantitative variables were expressed as absolute number and percentage and were compared using the chi-square test. Quantitative variables were expressed as mean (standard deviation [SD]) if they met the criteria of normality or median (interquartile range [IQR]) if they did not. They were compared using analysis of variance if they were parametric or the Kruskal–Wallis test if they were not parametric.</p><p id="par0080" class="elsevierStylePara elsevierViewall">To complete the descriptive analysis between patients with and without DM2, the correlations among malnutrition, sarcopenia, and comorbidity were analyzed for each group of patients with and without DM2 using Spearman's rank correlation coefficient. The results were represented on a heat map (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A and B).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">In regard to the prognostic analysis for in-hospital mortality, a logistic regression model was built that again included sarcopenia according to the dynamometry test, comorbidity according to the Charlson Comorbidity Index, inflammation according to the C-reactive protein/albumin ratio (CAR), and the predefined groups (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">Lastly, in order to observe 120-day prognosis, Kaplan-Meier curves were calculated for a primary outcome measure that included all-cause mortality or readmission and they were compared using the logrank test (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0095" class="elsevierStylePara elsevierViewall">A total of 511 patients were analyzed. Of them, 252 (49.1%) were men and the mean age was 75.2 ± 15 years. A total of 210 (41.1%) had DM2. The prevalence of malnutrition among patients with DM2 was 39.5% and the prevalence among those without DM2 was 39.4%. The characteristics of patients in the six predefined groups are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">In short, the groups of patients with DM2 (groups 4–6) were older and had a greater prevalence of other comorbidities associated with DM2 such as hypertension, dyslipidemia, heart failure, cerebrovascular disease, and pressure ulcers. The combination of DRM and sarcopenia was significant in groups 3 (<span class="elsevierStyleItalic">p</span> = 0.0001) and 6 and was particularly evident in the latter, according to the SARC-F and the dynamometry tests (<span class="elsevierStyleItalic">p</span> = 0.0003). A total of 63.8% of patients with diabetes had a score ≥4 on the SARC-F questionnaire compared to 49.5% of those without diabetes (<span class="elsevierStyleItalic">p</span> = 0.002). Likewise, the prevalence of sarcopenia based on the dynamometry test was 71.3% vs. 79.5% in individuals with and without DM2, respectively (<span class="elsevierStyleItalic">p</span> = 0.01). On the blood tests, significantly low serum transferrin, albumin, and hemoglobin values were notable among patients with DM2. In regard to inflammation, which was evaluated via CAR, group 6 presented with the greatest degree of inflammation (<span class="elsevierStyleItalic">p</span> = 0.008).</p><p id="par0105" class="elsevierStylePara elsevierViewall">According to the Spearman's rank correlation coefficient, significant associations were found among malnutrition, sarcopenia, comorbidity, and inflammation, especially in the case of patients with DM2 (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A and B).</p><p id="par0110" class="elsevierStylePara elsevierViewall">With respect to prognosis, there were no significant differences among groups in the incidence of in-hospital death. On the logistic regression model (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>), comorbidity, sarcopenia, and degree of inflammation significantly determined an increase in in-hospital mortality. In this model, group 6 (patients with DM2 and malnutrition) had a significantly lower mortality rate compared to group 1 (patients without DM2 or malnutrition) with OR 0.04, 95%CI 0.00−0.32, <span class="elsevierStyleItalic">p</span> = 0.01. Lastly, in regard to prognosis at 120 days, the group of patients with DM2 and malnutrition were those with the worst outcomes in terms of all-cause mortality and readmissions (<span class="elsevierStyleItalic">p</span> = 0.042, <a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">These results highlight the deleterious influence of the combination of DRM and sarcopenia on the prognosis of patients with DM2 hospitalized in the internal medicine ward. The presence of sarcopenia, defined as a low score on the dynamometry test (<span class="elsevierStyleItalic">p</span> = 0.056) or a score ≥4 on the SARC-F scale (<span class="elsevierStyleItalic">p</span> = 0.007) via inflammation (CAR) (<span class="elsevierStyleItalic">p</span> = 0.05) were the main determinants of in-hospital mortality. Previous studies have shown a direct relationship between in-hospital death in patients with malnutrition and a score ≥4 on the SARC-F test, mainly related to inability to climb stairs.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,24</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">No differences were found in in-hospital mortality according to malnutrition (<span class="elsevierStyleItalic">p</span> = 0.29), which was surprisingly lower in patients with DM2 and malnutrition (<span class="elsevierStyleItalic">p</span> = 0.01, <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). These findings may be due to chance, due to the sample size, or—most likely given this group’s characteristics of greater comorbidity and malnutrition—due to the fact that they may have received nutritional support and special care compared to patients in other groups. In regard to the probability of survival or readmission at 120 days, the prognosis was clearly unfavorable in patients with malnutrition (groups 3 and 6), especially if DM2 was also present (<span class="elsevierStyleItalic">p</span> = 0.042) (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p><p id="par0125" class="elsevierStylePara elsevierViewall">The prevalence of malnutrition among the patients was 39.5% and 39.4% in patients with and without DM2, respectively. In a multi-center observational study, Sanz Paris et al. reported a prevalence of nutritional risk in 39% and malnutrition in 21% of hospitalized adults with diabetes.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> In addition, the VIDA study on 159 patients with DM2 in internal medicine departments detected a prevalence of moderate malnutrition according to the GLIM criteria of 35.8% and severe malnutrition of 16.3%. The latter was associated with an increase in long-term mortality (OR 2.09; 95% CI 1.29–3.38), as was true in this study, which was mainly related to low serum albumin levels.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">DRM, like DM2, entails a deterioration in both muscle mass and function that is greater than what is to be expected from the reduction in muscle mass, especially after 60–70 years of age; the decline is related to muscle quality.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27,28</span></a> This is closely linked to systemic inflammation, insulin resistance, and fatty infiltration in the muscle.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">29,30</span></a> In this study, 79.5% of individuals with diabetes had low muscle strength, but so did 71.3% of patients without diabetes. The available evidence supports measuring muscle strength instead of total muscle mass in comorbid patients, such as those hospitalized in internal medicine departments, as recommended by the GLIM criteria and as supported in these results.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,19</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">In patients with DM2, the dynamometry score is significantly related to mortality, cardiovascular disease, other comorbidities, and fluctuations in glycemic control.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> Every 10-kg decline in muscle strength is associated with an increase in risk of mortality at 30 and 180 days.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">32,33</span></a> Various observational studies have demonstrated the deleterious effect sarcopenia has on prognosis in terms of morbidity and mortality as well as readmissions for patients with diabetes hospitalized due to different diseases.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">34–36</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Despite being a single-center study, this work’s findings can be extrapolated to the internal medicine department setting given the variety of patients included; the reliable reflection of patients commonly hospitalized in internal medicine wards; and the selection of patients at two different periods during the year, which limited the influence of seasonal trends in certain diseases.</p><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Limitations</span><p id="par0145" class="elsevierStylePara elsevierViewall">This study has several limitations. First, it is an observational study and thus there may be several variables that are not controlled for <span class="elsevierStyleItalic">a priori</span>. Given that this work is based on routine clinical practice, no data were available on any active nutritional treatment that may have been prescribed by the physician during hospitalization or follow-up. The anthropometric measurements of some patients with disabilities were estimated based on a validated formula, which may have underestimated them. This study also has several strengths, including the use of validated diagnostic tools for determining malnutrition and sarcopenia. In addition, the 120-day follow-up period was long enough to obtain solid results on the prognosis.</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusions</span><p id="par0150" class="elsevierStylePara elsevierViewall">The combination of DRM and sarcopenia at admission was associated with worse short- and long-term outcomes in patients with DM2 hospitalized in the internal medicine ward. The association among sarcopenia, comorbidity, and inflammation was especially relevant in in-hospital mortality. The factors that most influenced short-term prognosis were presence of sarcopenia measured by the SARC-F score and dynamometry as well as inflammation measured by CAR. The presence of both sarcopenia and malnutrition decreased the probability of readmission-free survival at 120 days. Therefore, early, proactive identification of malnutrition and especially of sarcopenia at the time of hospital admission are important elements in the care of patients hospitalized in internal medicine wards, especially patients with DM2.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conflicts of Malnourished patients with DM2 had significantly higher sarcopenia, comorbidity, inflammation, and pressure ulcers.
