Atrial fibrillation is the main reason for prescribing oral anticoagulation in our community.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a> The recent introduction of new oral anticoagulants (NOACs) that overcome the classic disadvantages of treatment with vitamin K antagonists (VKAs) and add clinical benefits in terms of morbidity and mortality (according to the various clinical trials that support them) has created a new clinical scenario at least as hopeful.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">2,3</span></a> Despite the evidence provided by these trials, there are still limited data on the use, safety and efficacy of NOACs in standard clinical practice. Recent studies have also shown poor anticoagulation control as measured by the time within the therapeutic range (TTR) using oral anticoagulation with VKAs in our community,<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">4,5</span></a> which reinforces the need to add data from standard practice on this issue. Our objectives, therefore, were to provide an overview of the use of NOACs and oral anticoagulation control with VKAs in an actual clinical scenario.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0010" class="elsevierStylePara elsevierViewall">A retrospective cohort analysis was conducted of patients hospitalized in a department of cardiology over a period of 3 years (from September 2011 to August 2014), with a primary or secondary diagnosis of atrial fibrillation and anticoagulant treatment at discharge. Of the 3112 consecutive hospitalizations, we identified 816 patients with atrial fibrillation (26.2%), 62 (7.6%) of whom had a prescription at discharge of NOACs. To assess the TTR with VKAs, we started by conducting a random selection (1:3), with a final analysis of 251 patients (of the total 754 patients). Thus, the measurement by number of INR controls (comparable to the Rosendaal method for determining the time in therapeutic range, using a cutoff of 60% instead of 65%)<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">3</span></a> found that 71.3% of the patients had a poor TTR. For this analysis, we reviewed the latest INR measurements during follow-up (minimum and maximum of 5 and 10, respectively, considering INR values <2 and >3 as a poor TTR). If an adverse event was recorded, the measurements prior to the event were considered.</p><p id="par0015" class="elsevierStylePara elsevierViewall">We divided the sample into 3 subgroups: patients undergoing treatment with NOACs, patients with VKAs and a good TTR and patients with VKAs and a poor TTR. In the survival analysis, we considered total mortality and the incidence of stroke (ischemic and hemorrhagic) as adverse events of efficacy. We also considered the incidence of acute myocardial infarction during follow-up as a significant clinical event, as reflected in the large clinical trials on NACO.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">6,7</span></a> We considered the incidence of stroke or myocardial infarction along with total mortality as a combined event. We assessed as a safety event the incidence of major bleeding, defined as a clinically significant hemorrhage that required a transfusion of at least 2 units of red blood cells, which required hospitalization or that caused death, according to the International Society on Thrombosis and Haemostasis criteria.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a> Strokes of hemorrhagic origin were excluded and were considered an efficacy event.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0020" class="elsevierStylePara elsevierViewall">To measure TTR, we employed the measurement by number of INR controls (comparable to the Rosendaal method for measuring TTR). Establishing the cutoff at 60% (instead of 65%<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">3</span></a>), 71.3% of the patients in our sample had poor TTR. These patients with poor TTR were younger. The groups were comparable in terms of cardiovascular risk factors and history of heart disease (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). A higher prevalence of hospitalization for heart failure was observed for the patients treated with VKAs versus NOACs (59% vs. 44%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.025) and, consequently, more frequent prescription of diuretics (82% vs. 65%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.003). The rate of hospitalization for arrhythmia and syncope was higher in the NOAC group (34% vs. 19%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.008). Higher creatinine levels were found in the patients prescribed VKAs (1.18 vs. 1.03, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.023), with comparable readings for the other laboratory parameters with prognostic impact. The systolic function determined by echocardiography was similar between the groups (left ventricular ejection fraction, 54.5% vs. 54.7%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.97).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">After assessing the scoring systems for embolic and hemorrhagic risk, we found a higher score on the HAS-BLED scale (1 point for each item: hypertension, abnormal renal/hepatic function, stroke, bleeding, labile INR, elderly (age<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>65), drugs/alcohol; patient considered at high hemorrhagic risk if score ≥3 points) for the patients with a poor TTR. This result was the consequence of lability in the INR values in this patient subgroup, included as a variable in the risk scale. Thus, no differences were found when performing the analysis by treatment group (patients with NOACs vs. VKAs, 2.90 vs. 3.02; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.48). The mean CHADS<span class="elsevierStyleInf">2</span> score (1 point for each item: congestive heart failure, hypertension, age ≥75, diabetes; and 2 points for stroke; for 2 or more items, anticoagulation is recommended) and CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc score (1 point for each item: congestive heart failure, hypertension, diabetes, vascular disease, age ≥65, sex category (female); and 2 points for age ≥75 and stroke; for ≥1 point, oral anticoagulation is recommended) were equivalent in the various subgroups.</p><p id="par0030" class="elsevierStylePara elsevierViewall">During a mean follow-up of 15 months, there was significantly lower mortality and fewer combined events for the patient group treated with NOACs than for the group treated with VKAs (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). These differences were especially significant when comparing the patients treated with NOACs with those treated with VKAs and poor TTR. Thus, a higher incidence of total mortality and combined events was observed in the patient group treated with VKAs and poor TTR compared with those treated with NOAC (mortality rate, 24% vs. 8%; combined events, 35% vs. 16%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.01 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.02, respectively). There was a nonsignificant tendency compared with the group with good INR control (mortality rate, 25% vs. 8%; combined events, 35% vs. 16%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.29 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.05, respectively), although the follow-up time in the various subgroups differed significantly and was shorter in the NOAC group. Additionally, a favorable (although not significant) tendency for NOACs was found when the incidence of stroke and major bleeding was compared (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The Kaplan–Meier survival curves for the combined event showed a tendency toward longer event-free time in the group treated with NOACs (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">The results of this study suggest, first, that the rate of NOAC prescription is very low (<10%), lower than expected based on the cost-effectiveness studies performed in our community.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a> We also confirmed the poor TTR when continuing treatment with VKAs (>70% of the patients), in our case greater than that of previous registries,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">4</span></a> probably due to the fact that this is a population with previous heart disease and greater comorbidity. The increased incidence of stroke and systemic embolism associated with poor TTR with VKAs and the costs resulting from the use of NOACs have recently been reviewed based on the results of the major clinical trials ARISTOTLE, ROCKET-AF and RE-LY.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">10</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Although the subgroups are not comparable due to the study design, with a higher prevalence of patients with heart failure and poorer renal function in those treated with VKAs and different follow-up periods, the survival analysis seems to confirm the safety and efficacy of the use of NOACs in clinical practice, with no new adverse effects or undesirable events. The first published meta-analysis with patients in the 4 clinical trials with NOACs (including those of ENGAGE AF-TIMI 48 with edoxaban)<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">11</span></a> appears to confirm the favorable risk profile of NOACs, with a reduced incidence of stroke, intracranial hemorrhaging and total mortality and a similar risk of major hemorrhages. This effect also appears to extend to a broad spectrum of patients.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The limitations of a single-center, retrospective registry are assumed, with an assessment of anticoagulation control using INR measurements and follow-up analysis using electronic medical histories, which can result in the loss of events in the registry. However, the study could meet its objective of providing a general overview of the state of oral anticoagulation in patients previously hospitalized in a cardiology department of a tertiary hospital.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusions</span><p id="par0050" class="elsevierStylePara elsevierViewall">For patients with a previous hospitalization in cardiology in a tertiary hospital and a primary or secondary diagnosis of atrial fibrillation, the TTR with VKAs was poor. New oral anticoagulants (NOACs) overcome the disadvantages of vitamin K antagonists (VKAs), although there are scarce data on its use in our community. The aim of our study was to assess the use of NOACs and anticoagulation control using VKA as measured by the time within the therapeutic range (TTR) in an actual clinical scenario.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A retrospective cohort analysis was conducted of 816 patients admitted to cardiology over a period of 3 years, with a diagnosis of atrial fibrillation and anticoagulant treatment at discharge. We assessed the percentage of patients prescribed NOACs and the TTR with VKA. We compared safety and efficacy events during the 15-month follow-up among the patients prescribed NOAC, those prescribed VKA with a good TTR and those with a poor TTR.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The percentage of patients prescribed NOAC was 7.6%. Serial INR measurements found that 71.3% of patients had a poor TTR. Se compararon eventos de seguridad y eficacia durante un seguimiento de 15 meses entre los pacientes con NACO, los pacientes con AVK y buen CRT, y aquellos con mal CRT.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">El porcentaje de prescripción de NACO fue del 7,6%. La determinación seriada de INR encontró un 71,3% de pacientes con mal CRT.
