El objetivo del tratamiento en estadios III A y III B sin derrame pleural debe ser tanto el control locorregional como la enfermedad a distancia. Sabemos que al menos el 80% de los pacientes presentan enfermedad micrometastásica en el momento del diagnóstico, por lo que un tratamiento únicamente local estaría condenado al fracaso desde el mismo momento de plantearlo. Continúan abiertos muchos interrogantes clínicos y cada paso conseguido va seguido de una nueva disyuntiva diagnóstica o terapéutica y, por todo ello, constituye un campo abonado para la investigación clínica.

The objective of the treatment in stages III A and III B without pleural effusion should be both locoregional and distant disease control. We known that at least 80% of the patients had micrometastatic disease at the time of diagnosis, so that a treatment that is only local would be condemned to failure from the time it is suggested. Many clinical questions continue to exist and each step achieved is followed by a new diagnostic or therapeutic dilemma and is thus a fertile field for clinical research.
