Alzheimer’s disease and periodontal disease: uncovering the association through systematic review and meta-analysis

Abstract
Objective

This meta-analysis aimed to determine the association between periodontal disease (PD) and Alzheimer’s disease (AD).

Methods

Observational studies reporting the association between PD and AD published from inception to March 2025 in PubMed, Scopus, EBSCO, ScienceDirect, Web of Science, and CENTRAL were searched. The ROBINS-I tool was used for risk of bias assessment. Meta-analysis was performed using a random-effects model and presented as odds ratio (OR) with 95% confidence interval (CI). Analyses were run using Review Manager 5.4.

Results

Six studies (n = 291,057 participants) met the inclusion criteria. Pooled analysis showed a significant association between PD and AD (OR = 2.67; 95% CI: 1.41–5.06; p =  0.003; I2 = 92%). Sensitivity analysis showed a consistent direction of association. The substantial heterogeneity likely reflects differences in study design and confounder adjustment across studies.

Conclusions

This study indicates a potential association between PD and AD. However, given the limited number of studies, substantial heterogeneity, and the possibility of residual confounding, the pooled estimate should be interpreted with caution. Well-designed prospective and interventional studies are needed to clarify the potential causality.

Objetivos

Este metaanálisis tuvo como objetivo determinar la asociación entre la enfermedad periodontal (EP) y la enfermedad de Alzheimer (EA).

Métodos

Se buscaron estudios observacionales que reportaran la asociación entre la EP y la EA, publicados desde su inicio hasta marzo de 2025 en PubMed, Scopus, EBSCO, ScienceDirect, Web of Science y CENTRAL. Se utilizó la herramienta ROBINS-I para la evaluación del riesgo de sesgo. Se llevó un metaanálisis utilizando un modelo de efectos aleatorios, cuyos resultados se expresaron como razón de momios (OR) con un intervalo de confianza (IC) del 95%. Los análisis se realizaron utilizando Review Manager 5.4.

Resultados

Seis estudios (n = 291.057 participantes) cumplieron los criterios de inclusión. El análisis agrupado mostró una asociación significativa entre la EP y la EA (OR = 2,67; IC del 95 %: 1,41–5,06; p =  0,003; I2 = 92 %). El análisis de sensibilidad mostró una dirección de asociación consistente. La heterogeneidad sustancial probablemente refleja diferencias en el diseño de los estudios y en el ajuste de factores de confusión.

Conclusiones

Este estudio indica una asociación potencial entre la EP y la EA. Sin embargo, dado el número limitado de estudios, la heterogeneidad sustancial y la posibilidad de confusión residual, la estimación agrupada debe interpretarse con cautela. Se necesitan estudios prospectivos e intervencionistas bien diseñados para aclarar la posible causalidad.
